<a href="https://www.facebook.com/share/p/1Gj6RCQQxh/">شاركت</a> العديد من النساء من مختلف أنحاء مصر شهادات مروعة عن تجاربهن أثناء الولادة عبر وسائل التواصل الاجتماعي. وذلك عقب إلقاء القبض على طبيبة في 16 يونيو بسبب كشفها الانتهاكات التي شهدتها أثناء فترة عملها كطبيبة امتياز في مستشفى الشاطبي بالإسكندرية. ويدفعنا الرد الرسمي، أو بالأحرى قصوره، إلى التأكيد على ضرورة وضع حد للعنف الموجه ضد النساء.

<h3 dir="rtl">العنف التوليدي: أحد أشكال الاعتداءات المتعددة الموجهة ضد النساء</h3>
تمثل الانتهاكات التي تتعرض لها النساء أثناء عمليات الولادة نموذجًا لما يُعرف بـ العنف التوليدي، والذي يرتبط ارتباطًا وثيقًا بالمعايير الجندرية السائدة والتمييز المتوارث ضد النساء في المجتمع. <a href="https://www.law.georgetown.edu/georgetown-law-journal/wp-content/uploads/sites/26/2018/06/Obstetric-Violence.pdf">ويشمل</a> العنف التوليدي أي شكل من أشكال الإساءة - الجسدي أو اللفظي - وتوجيه الإهانات للنساء أثناء الحمل والولادة وفترة رعاية ما بعد الولادة. كما يشمل دفع&nbsp; النساء إلى اتخاذ قرارات غير مستنيرة بشأن خيارات الولادة المتاحة لهن - مثل <a href="https://aps.aucegypt.edu/ar/articles/1487/podcast-el-hal-eh-with-dr-rabab-el-mahdi-discusses-womens-health-in-egypt">الخضوع</a>&nbsp; للعمليات القيصرية غير الضرورية. <a href="https://aps.aucegypt.edu/ar/articles/750/birth-trauma-and-maternal-mental-health-in-egypt">ويُطلق</a> بعض الباحثين على هذه الظاهرة مصطلح "صدمة الولادة"، نظرًا لأن آثار تلك الانتهاكات قد تصبح طويلة الأمد وتؤثر على حياة النساء بعد حدوثها بمدة طويلة. ولذا، لا تقتصر المسؤولية بالضرورة على الأفراد وحدهم، الأمر الذي يجعل معالجة هذه الانتهاكات والاعتراف بحدوثها أمرًا بالغ الصعوبة، وهو ما تجلى بوضوح في رد فعل الدولة الرسمي <a href="https://www.facebook.com/share/p/1GPRXKa4Bd/">تجاه</a> الطبيبة التي كشفت النقاب عنها.

<h3 dir="rtl">ما مدى انتشار العنف التوليدي في مصر؟</h3>
تُمثل إفادات الضحايا المصدر الوحيد الموثوق للمعلومات في ظل غياب البيانات الإحصائية الرسمية. وقد اتسمت هذه الإفادات أو الشهادات بالاتساق والتواتر على مدار السنوات، رغم محدودية عددها نتيجة للوصمة الاجتماعية والترهيب الرسمي. وتظهر <a href="https://doi.org/10.1016/j.rhm.2016.04.002">الدراسات</a> القليلة المتاحة أن مصر تعاني تراجعًا حادًا في جودة الرعاية الطبية القائمة على الموافقة المستنيرة. ويعد إجراء العمليات القيصرية بناءً على قرارات غير مستنيرة أمرًا شائعًا للغاية، على الرغم من أنها تمثل تدخلًا طبيًا غير ضروري في كثير من الحالات. وفي كثير من الأحيان يفرضها الأطباء على المريضات بدافع الاستسهال، أو لتحقيق مكاسب مالية، وأحيانا بسبب ضعف الكفاءة الطبية.&nbsp;

كما توثق الدراسات معدلات مرتفعة من التدخلات الطبية غير الضرورية الأخرى، مثل الضغط المتكرر على الرحم أثناء الولادة باعتباره إجراء روتيني، <a href="https://doi.org/10.3389/fgwh.2022.850796">وتحفيز المخاض</a> اصطناعيًا لتسريع عملية الولادة رغم اتساع عنق رحم الأم، وتكرار الفحوصات المهبلية التي تسبب صدمة شديدة للنساء. كما لا يسمح بوجود مرافقين للمريضات أثناء عملية الولادة، كما يتم تقييد حركتهن إلى الأسرة في بعض الأحيان.

<h3 dir="rtl">سبل منع حدوث هذه الانتهاكات</h3>
يتعين على الدولة البدء في سلسلة من الإجراءات الإصلاحية الشاملة، التي تجمع بين التدابير الوقائية ومعالجة الانتهاكات التي تحدث بالفعل، وذلك لضمان التصدي لهذه الممارسات بشكل حاسم. ويتطلب الأمر، على <a href="https://doi.org/10.1186/s12884-015-0645-6">مستوى</a> السياسات العامة، اعترافًا صريحًا من جميع الأطراف المعنية بوجود هذه الانتهاكات، مع تبني نهج قائم على عدم التسامح مطلقًا مع تكرارها مستقبلًا. وفي هذا السياق، يجب على الحكومة إدراج ممارسات العنف التوليدي ضمن أي سياسة معتمدة لمكافحة العنف القائم على النوع الاجتماعي، وتوسيع نطاق مفهوم الموافقة المستنيرة ليشمل الممارسات الروتينية مثل الفحوصات المهبلية. كما ينبغي على <a href="https://eeca.unfpa.org/en/publications/respectful-maternity-care-womens-experiences-and-outlooks-eastern-europe-and-central">الحكومة</a> إنشاء آليات مستقلة لضمان التحقيق في هذه الانتهاكات ورد الاعتبار للضحايا، مع توفير الحماية والسرية التامة لهن وللمبلّغين. ويتوجب على السلطات اتخاذ إجراءات تأديبية رادعة لمكافحة العنف التوليدي تتناسب مع جسامة الفعل، بدءًا من العقوبات المهنية ووصولًا إلى الملاحقة القضائية. وبالإضافة إلى ذلك، يجب إرساء آليات فعالة للمساءلة تعتمد على تتبع البيانات مثل معدلات السماح بدخول المرافقين ونسب إجراء شق العجان، بالإضافة إلى القيام بمراجعات مؤسسية دورية لرصد أنماط الانتهاكات ومدى التقدم الذي تم إحرازه.

Women across Egypt took to social media to <a href="https://www.facebook.com/share/p/1Gj6RCQQxh/">share</a> horrific accounts of their birth deliveries after a former residential doctor was arrested on June 16 for sharing abuses that she witnessed in Shatbi Hospital in Alexandria. The official response, or lack thereof, underscores our urgency to de-normalize complacency with violence against women.

<h3 dir="ltr">Obstetric violence: One of many aggressions against women</h3>
The mistreatment of women during childbirth is closely linked with a society&rsquo;s gender norms and inherent discrimination against them. Obstetric violence <a href="https://www.law.georgetown.edu/georgetown-law-journal/wp-content/uploads/sites/26/2018/06/Obstetric-Violence.pdf">includes</a> any type of abuse (physical or verbal) and disrespect during pregnancy, childbirth, and postnatal care, as well as coercing women to make uninformed decisions about their birth choices (such as <a href="https://aps.aucegypt.edu/en/articles/1487/podcast-el-hal-eh-with-dr-rabab-el-mahdi-discusses-womens-health-in-egypt">unnecessary</a> cesarean operations). Some scholars <a href="https://aps.aucegypt.edu/en/articles/750/birth-trauma-and-maternal-mental-health-in-egypt">call this</a> birth trauma because the effect of these violations can be long-lasting, impacting women&rsquo;s lived experiences long after the incident. Since such treatment often stems from systemic factors, culpability is not necessarily individual, which makes it much harder to address and acknowledge, as evidenced by the state&rsquo;s response <a href="https://www.facebook.com/share/p/1GPRXKa4Bd/">to</a> the doctor who reported the violations.&nbsp;

Unlike other types of violence against women, obstetric violence is often medicalized and therefore invisible to both women and the <a href="https://www.law.georgetown.edu/georgetown-law-journal/wp-content/uploads/sites/26/2018/06/Obstetric-Violence.pdf">legal</a> system. In medical settings, doctors have <a href="https://doi.org/10.1016/j.rhm.2016.04.002">authority</a> over women&mdash;they are professionals who need to make life-or-death decisions in moments of emergency&mdash;thereby inhibiting women&rsquo;s willingness to resist or question certain practices. At the same time, many women are not fully aware of their rights to <a href="https://www.bibalex.org/baifa/Attachment/Documents/HzKxSFxehc_20210714162426264.pdf">question</a> medical decisions. Abuses are therefore often brushed off as insignificant or even &ldquo;normal&rdquo; protocol.&nbsp;
<h3 dir="ltr">How often does this occur in Egypt?</h3>
In light of significant information gaps, witness accounts remain the only reliable source. These have been consistent over the years, although social stigma and official intimidation may prevent many women from reporting mistreatment. The few <a href="https://www.bibalex.org/baifa/Attachment/Documents/HzKxSFxehc_20210714162426264.pdf">studies</a> that exist show that Egypt rates poorly in consented and informed care. Cesarean operations are commonplace and based on uninformed decisions even though they are an unnecessary medical intervention. Doctors often force them on their patients out of convenience, financial incentives, and sometimes poor medical knowledge.&nbsp;

Research has also documented high occurrences of other types of unnecessary medical interventions, such as routine uterine pressure during deliveries, birth <a href="https://doi.org/10.3389/fgwh.2022.850796">inductions</a> to speed up delivery even after dilation begins, and frequent vaginal exams, which cause women severe trauma. Women are also not allowed a companion during delivery and are at times physically confined to their beds.

<h3 dir="ltr">How do we stop this from happening?</h3>
Egypt must instigate a host of reforms, some preventive and others after the fact, to ensure these types of violations are adequately addressed. At the policy <a href="https://doi.org/10.1186/s12884-015-0645-6">level</a>, there needs to be a clear acknowledgment by all stakeholders that the problem exists and will no longer be tolerated. At a minimum, the government must ensure that acts of obstetric violence are included in any gender-based violence policy they adopt. This must also include expanded parameters for consent to include &ldquo;routine&rdquo; practices such as vaginal examinations. The <a href="https://eeca.unfpa.org/en/publications/respectful-maternity-care-womens-experiences-and-outlooks-eastern-europe-and-central">government</a> must also establish mechanisms to ensure that abuses are investigated and redressed by independent bodies, with protection and anonymity for both the victim and any whistleblower. Authorities must make disciplinary measures clear to all to address obstetric violence, and ensure that these measures are commensurate with the gravity of the act (ranging from professional sanction to prosecution). The government must also ensure that accountability mechanisms are in place, relying on data tracking (companion log-in rates, episiotomy rates) and institutional audits to identify patterns and measure progress.

Most importantly, the foundation for preventing such violations begins with properly training medical professionals, both technically and ethically, to understand the gravity of these violations, while fostering working conditions that militate against such abuses. In addressing the problem, the key need is not necessarily more resources but the political will to end the normalization of violence against women.
