<h3 dir="rtl">انخفاض قيمة الإنفاق الحكومي على الصحة</h3>
لا <a href="https://manshurat.org/node/4256">يلتزم</a> الإنفاق الحكومي على الصحة بالنص الدستوري (مادة 18) بضرورة تخصيص 3% من الإنتاج المحلي الإجمالي، فلا تتعدى النسبة الحالية 1.2% في 2024/2023. كما أن نسبة الإنفاق على الصحة من إجمالي الإنفاق الحكومي مقارنة بين عامي 2020/2019 و2024/2023 انخفض من 6% إلى 4.9%. بالإضافة إلى ذلك، ارتفع معدل التغير السنوي للتضخم في قسم الرعاية الصحية مقارنة بمعدل الزيادة في الإنفاق الحكومي على الصحة 2024/2023.

وسيكون لتعديلات اللائحة تأثير كبير في المرضى الفقراء، حيث أقرت منح المرضى غير القادرين خصمًا بحد أقصى 30% من تكاليف الخدمات العلاجية بعد استبعاد قيمة الأدوية والمستلزمات الطبية وتخفيض نسبة العلاج المجاني لنحو 25% من عدد الأسِّرَّة بعد أن كانت 60% سابقًا، وزيادة سعر تذكرة الكشف والعلاج من جنيه إلى 10 جنيهات، و<a href="https://www.vetogate.com/5106847">منح</a> المريض دواءً أساسيًّا واحدًا فقط، وتخفيض 15% على باقي العلاج، بعد أن كان في السابق يحصل على العلاج كله مجانيًّا.
<h3 dir="rtl">الإتاحة في القطاع الصحي الحكومي&nbsp;</h3>
في الوقت الذى بلغ فيه معدل <a href="https://aps.aucegypt.edu/ar/events/110/open-panel-discussion-at-aps-on-poverty-figures-in-egypt-an-inflation-driven-increase">الفقر</a> نحو 35.7% في يوليو عام 2023 ما يعادل 37.5 مليون فقير، انعكس ضعف الإنفاق الحكومي على الصحة على حجم <a href="https://aps.aucegypt.edu/ar/articles/786/health-equity-in-egypt-reflections-in-2022">الخدمات</a> الصحية المتاحة للمرضى الفقراء، فعلي سبيل المثال <a href="https://www.capmas.gov.eg/Pages/Publications.aspx?page_id=5104&amp;Year=23361">انخفض</a> عدد الأسِّرَّة بالمستشفيات الحكومية، خلال 10 سنوات بنحو 13.8 ألف سرير.
<h3 dir="rtl">سياسات تقلل من آثار رفع أسعار الخدمات الصحية</h3>
تشير التوقعات إلى بلوغ إجمالي سكان مصر 130 مليون نسمة بحلول عام 2030، ورغم ارتفاع نسبة الفئات العمرية الأكثر طلبًا للرعاية الصحية، مثل فئة رعاية الطفولة والأمومة، و<a href="https://gate.ahram.org.eg/News/2572404.aspx">انتشار</a> الأمراض المزمنة، والسمنة، بالإضافة إلى رعاية المسنين، فإنه من المهم تنفيذ سياسات تسهِّل من حصول المرضى خاصة الفقراء منهم، على الخدمات الصحية بسهولة وأقل تكلفة:&nbsp;

<h2 dir="rtl">رفع سعر إردب القمح لا يحقق الاكتفاء الذاتي</h2>
<a href="https://www.cabinet.gov.eg/News/Details/75420">وافق</a> مجلس الوزراء المصري على رفع سعر توريد إردب القمح إلى 2000 جنيه لموسم 2025/2024، ما يوازي 41.32 دولارًا، لتشجيع الفلاحين على زراعة القمح. ورغم أن الموافقة جاءت بناء على <a href="https://www.skynewsarabia.com/business/1699572-%D9%85%D8%B5%D8%B1-%D8%B1%D9%81%D8%B9-%D8%B3%D8%B9%D8%B1-%D8%AA%D9%88%D8%B1%D9%8A%D8%AF-%D8%A7%D9%84%D9%82%D9%85%D8%AD-%D8%A7%D9%84%D9%85%D8%AD%D9%84%D9%8A-4132-%D8%AF%D9%88%D9%84%D8%A7%D8%B1">طلب</a> المزارعين في نوفمبر 2023، فإنها تأخرت وتزامنت مع قرار تعويم الجنيه الأخير ما يجعلها فارغة المضمون.&nbsp;
تأتي هذه الخطوة في ظل <a href="https://www.elbalad.news/5905005#:~:text=%D9%88%D9%85%D9%86%20%D8%AC%D8%A7%D9%86%D8%A8%D9%87%D8%8C%20%D9%82%D8%A7%D9%84%20%D8%A7%D9%84%D8%AF%D9%83%D8%AA%D9%88%D8%B1%20%D8%AC%D9%85%D8%A7%D9%84,%D9%86%D8%B3%D8%A8%D8%A9%20%D8%A7%D9%84%D8%A7%D9%83%D8%AA%D9%81%D8%A7%D8%A1%20%D8%A7%D9%84%D8%B0%D8%A7%D8%AA%D9%8A%20%D9%85%D9%86%20%D8%A7%D9%84%D9%82%D9%85%D8%AD.">خطط</a> الحكومة للوصول إلى 70% اكتفاء ذاتي من القمح بحلول 2030، وهو هدف من الصعب <a href="https://www.mdpi.com/2077-0472/13/1/7">تحقيقه</a> مع تحديات الوضع الاقتصادي داخليًّا وعالميًّا، ومعدلات النمو السكاني وعدم اتساق زيادة الإنتاج المحلي معها.

<h3 dir="rtl">هل تستطيع مصر الاكتفاء ذاتيًّا من القمح؟</h3>
يعد تقلب سعر القمح عالميًّا بسبب الأزمات الدولية وتحديات سلاسل الإمداد، وبالتالي ارتفاع فاتورة الاستيراد، هو <a href="https://www.frontiersin.org/articles/10.3389/fsufs.2023.1137526/full">المحرك</a> الأساسي وراء هدف الاكتفاء الذاتي. وتسعى الحكومة إلى زيادة الرقعة المزروعة قمحًا من خلال تشجيع الفلاحين بزيادة أسعار التوريد أو من خلال مشاريع الاستصلاح الزراعي. وأصدرت قرارًا بحد أقصى لنسبة استيراد القمح 5% من فاتورة الواردات. وبالفعل، ارتفع متوسط إنتاج مصر السنوي من القمح بـ 1.9% خلال الفترة من 2000 إلى 2020. وبينما نجحت مصر في زيادة إنتاجية الفدان المزروع قمحًا اعتمادًا على تكنولوجيات الهندسة الزراعية، فإن هذه الإنتاجية تتسابق مع معدلات النمو السكاني وتوقعات وصول عدد السكان إلى 140 مليونًا بحلول 2050.

ليس المعدل السكاني وحده هو المؤثر في عدم القدرة على تحقيق الاكتفاء الذاتي، بل معدل استهلاكنا من القمح. وتأتي مصر في المركز السادس عالميًّا في الاستهلاك بعد دول أخرى ذات حجم سكاني أكبر.&nbsp;
تواجه الخطة الحكومية للاكتفاء الذاتي عدة عقبات، منها: <a href="https://www.lse.ac.uk/granthaminstitute/news/climate-change-food-consumption-and-child-health-in-egypt-how-can-outcomes-be-improved/">ارتفاع</a> تكلفة زراعة القمح محليًّا بسبب الشح المائي، وارتفاع درجات الحرارة وتأثيرات <a href="https://carnegieendowment.org/2023/10/26/climate-change-in-egypt-opportunities-and-obstacles-pub-90854">التغيرات المناخية</a>. بالإضافة إلى أنه نتيجةً للتضخم <a href="http://www.arc.sci.eg/Pub_Details.aspx?TabId=1&amp;NavId=2&amp;PUB_ID=796341&amp;OrgID=&amp;lang=ar#:~:text=%D8%B2%D8%A7%D8%AF%D8%AA%20%D8%AA%D9%83%D8%A7%D9%84%D9%8A%D9%81%20%D8%A5%D9%86%D8%AA%D8%A7%D8%AC%20%D9%81%D8%AF%D8%A7%D9%86%20%D8%A7%D9%84%D9%82%D9%85%D8%AD,%D8%A7%D9%84%D8%A2%D9%84%D8%A7%D8%AA%20%D9%88%D8%A7%D9%84%D8%B3%D9%85%D8%A7%D8%AF%20%D8%A7%D9%84%D9%83%D9%8A%D9%85%D8%A7%D9%88%D9%8A%20%D9%86%D8%AD%D9%88%2050.8%25">ارتفعت</a> تكاليف زراعة فدان القمح من 7.5 آلاف جنيه في 2021/2020 إلى 9.9 آلاف في العالم التالي، ووصلت تقديرات التكلفة إلى 17 ألف جنيه في 2023/2022.
وفي ظل <a href="https://aps.aucegypt.edu/ar/articles/1017/fdi-and-wheat-gap-challenges">الفجوة القمحية</a> المقدرة بـ6.5% خلال الـ20 سنة السابقة، من المرجح أ<a href="https://www.frontiersin.org/articles/10.3389/fsufs.2023.1137526/full">لا تستطيع</a> مصر الوصول إلى نسبة <a href="https://jaesj.journals.ekb.eg/article_236693.html">أعلى</a> من 50% من الاكتفاء الذاتي من القمح.&nbsp;

<h3 dir="rtl">هل الاكتفاء الذاتي هو الحل الأمثل؟&nbsp;</h3>
بعد رفع سعر التوريد <a href="https://gate.ahram.org.eg/daily/News/204800/136/936123/%D9%85%D8%AA%D8%A7%D8%A8%D8%B9%D8%A7%D8%AA/%D8%BA%D8%B1%D9%81%D8%A9-%D8%A7%D9%84%D8%AD%D8%A8%D9%88%D8%A8--%D9%84%D8%A3%D9%88%D9%84-%D9%85%D8%B1%D8%A9-%D8%B3%D8%B9%D8%B1-%D8%A7%D9%84%D9%82%D9%85%D8%AD-%D8%A7%D9%84%D9%85%D8%AD%D9%84%D9%89-%D8%A3%D8%B9%D9%84%D9%89-%D9%85%D9%86-%D8%A7%D9%84%D9%85.aspx">أصبح</a> سعر طن القمح المحلي 13.3 ألف جنيه، بينما يتراوح سعر طن القمح المستورد من 12.2 إلى 12.5 ألف جنيه. وهذا نتيجة لسياسة الدولة الممتدة لسنوات في زيادة سعر التوريد؛ حيث وصل الفارق في السعر إلى 84 دولارًا زيادة لصالح <a href="https://www.fao.org/3/i4898e/i4898e.pdf">القمح المحلي</a> عام 2014. ويظل السعي وراء زيادة التوريد المحلي من القمح مهمًّا لضمان درجة من <a href="https://aps.aucegypt.edu/ar/articles/1253/food-security-and-green-hydrogen-who-benefits">الأمن الغذائي</a> في مصر، خصوصًا في ظل انخفاض نصيب الفرد من <a href="https://aps.aucegypt.edu/ar/articles/1368/lack-of-transparency-and-protein">البروتين النباتي</a> والذي يعد <a href="https://aps.aucegypt.edu/ar/articles/1017/fdi-and-wheat-gap-challenges">القمح</a> مصدرًا رئيسًا له، <a href="https://www.frontiersin.org/articles/10.3389/fsufs.2020.617009/full">واعتماد</a> الفئات الفقيرة عليه بسبب وفرته والانخفاض النسبي في أسعاره نتيجة الدعم الحكومي له.&nbsp;

ورغم أهمية تحقيق الاكتفاء الذاتي من القمح، إلا أنه يجب الموازنة بين تحقيق نسب اكتفاء معقولة، مع عدم إهمال السلع الأخرى التي تنتجها مصر. وتعتبر الخضروات من هذه المنتجات التي يساهم توفيرها بأسعار مناسبة في تشجيع المواطنين على تغيير أنماط تغذيتهم وبالتالي تقليل الاعتماد على القمح.

With the health sector struggling with a dearth of resources, the change aims at increasing out-of-pocket spending by patients. Although in absolute numbers, total government spending on the health sector has increased, the value of expenditure has fallen with changes in the exchange rate and rising inflation.

<h3>Falling value of government health expenditure</h3>
Government spending on health does not comply with <a href="https://manshurat.org/node/4256">Article 18</a> of the Egyptian constitution, which requires 3% of GDP to be channeled to health; in 2023/24, the state spent no more than 1.2% of GDP on health. In addition, government health spending as a percentage of total government spending decreased from 6% in 2019/20 to 4.9% in 2023/24. Furthermore, increases in the cost of health care due to inflation outpaced increases in government health spending in 2023/24.&nbsp;

The regulatory changes will have a significant impact on poor patients. They cap the discount on treatment for needy patients at 30%, exclusive of medication costs and medical supplies, and reduce the number of beds set aside for free treatment from 60% of all beds to 25%. The new regulations also raise the price of an examination and treatment ticket from EGP1 to EGP10, while&nbsp;<a href="https://www.vetogate.com/5106847">covering</a> just one basic medication per patient and offering a 15% discount on the remaining treatment. Previously, patients received all treatment free of charge.
<h3 dir="ltr">Availability in the government health sector</h3>
As the <a href="https://aps.aucegypt.edu/en/events/110/open-panel-discussion-at-aps-on-poverty-figures-in-egypt-an-inflation-driven-increase">poverty rate</a> has climbed&mdash;reaching about 35.7% in July 2023, the equivalent of 37.5 million people&mdash;inadequate government spending on health has been reflected in the volume of <a href="https://aps.aucegypt.edu/en/articles/786/health-equity-in-egypt-reflections-in-2022">health services</a> available to poor patients. For example, the number of beds in government hospitals <a href="https://www.capmas.gov.eg/Pages/Publications.aspx?page_id=5104&amp;Year=23361">fell</a> by some 13,800 in a single decade.
<h3 dir="ltr">Policies to mitigate the impact of higher priced health services</h3>
Egypt&rsquo;s population is projected to reach 130 million by 2030, entailing an increase in the population groups most in need of health care, such as mothers and children and the elderly, and this as chronic illness such as obesity are <a href="https://gate.ahram.org.eg/News/2572404.aspx">on the rise</a>. It is therefore vital to implement policies that facilitate access to affordable health services, especially for poor patients:

<h2 dir="ltr">Increasing Wheat Procurement Prices Will Not Achieve Self-Sufficiency</h2>
The Egyptian Cabinet <a href="https://www.cabinet.gov.eg/News/Details/75420">approved</a> an increase in the supply price of wheat to EGP2,000/ardeb for the 2024/25 season, the equivalent of USD41.32, to encourage farmers to grow the grain. Farmers <a href="https://english.ahram.org.eg/NewsContent/1/2/519423/Egypt/Society/Egypt-raises-wheat-procurement-price-by--in-new-ha.aspx#:~:text=Egypt's%20cabinet%20has%20increased%20the,for%20the%20forthcoming%20harvest%20season.&amp;text=The%20decision%20is%20meant%20to,the%20cabinet%20spokesperson%2C%20on%20Wednesday">had asked</a> for the price increase in November 2023, but approval was delayed. Now the recent devaluation of the pound has eroded the value of the increase.
This move comes as part of the government&rsquo;s <a href="https://www.egypttoday.com/Article/3/122821/Egypt-aims-to-produce-70-of-wheat-locally-by-2030">plan</a> to be 70% self-sufficient in wheat by 2030, but it will be difficult to <a href="https://www.mdpi.com/2077-0472/13/1/7">hit this target</a> considering domestic and global economic challenges and the fact that the population is growing at a rate higher than domestic wheat production.

<h3 dir="ltr">Can Egypt attain self-sufficiency in wheat?</h3>
The main <a href="https://www.frontiersin.org/articles/10.3389/fsufs.2023.1137526/full">impetus</a> for the drive to self-sufficiency is the volatility of global wheat prices due to international crises and supply chain disruption, which, in turn, means a higher import bill. The government is aiming to increase the area cultivated with wheat by encouraging farmers through either increased supply prices or agricultural reclamation projects. It also issued a decree capping wheat imports at 5% of total import bill. Overall, average annual wheat production increased 1.9% from 2000 to 2020. While Egypt has succeeded in increasing productivity per feddan thanks to agricultural engineering technologies, the gains in productivity are being outstripped by population growth rates, amid projections that the population will reach 140 million by 2050.

Population growth is not the only factor standing in the way of self-sufficiency. Wheat consumption is another impediment. Egypt ranks sixth in the world in consumption, coming after other countries with larger population.
The government&rsquo;s self-sufficiency plan faces several obstacles, among them the <a href="https://www.lse.ac.uk/granthaminstitute/news/climate-change-food-consumption-and-child-health-in-egypt-how-can-outcomes-be-improved/">high cost</a> of growing wheat locally due to water scarcity, high temperatures, and the impact of <a href="https://carnegieendowment.org/2023/10/26/climate-change-in-egypt-opportunities-and-obstacles-pub-90854">climate change</a>. In addition, as a result of inflation, the cost of growing a feddan of wheat <a href="http://www.arc.sci.eg/Pub_Details.aspx?TabId=1&amp;NavId=2&amp;PUB_ID=796341&amp;OrgID=&amp;lang=ar#:~:text=%D8%B2%D8%A7%D8%AF%D8%AA%20%D8%AA%D9%83%D8%A7%D9%84%D9%8A%D9%81%20%D8%A5%D9%86%D8%AA%D8%A7%D8%AC%20%D9%81%D8%AF%D8%A7%D9%86%20%D8%A7%D9%84%D9%82%D9%85%D8%AD,%D8%A7%D9%84%D8%A2%D9%84%D8%A7%D8%AA%20%D9%88%D8%A7%D9%84%D8%B3%D9%85%D8%A7%D8%AF%20%D8%A7%D9%84%D9%83%D9%8A%D9%85%D8%A7%D9%88%D9%8A%20%D9%86%D8%AD%D9%88%2050.8%25">increased</a> from EGP7,500 in 2020/21 to EGP9,900 the following year to an estimated EGP17,000 in 2022/23.
In light of an estimated <a href="https://aps.aucegypt.edu/en/articles/1017/fdi-and-wheat-gap-challenges">wheat gap</a> of 6.5% over the previous 20 years, Egypt will likely be <a href="https://www.frontiersin.org/articles/10.3389/fsufs.2023.1137526/full">unable</a> to <a href="https://jaesj.journals.ekb.eg/article_236693.html">exceed</a> a wheat self-sufficiency of 50%.&nbsp;

<h3 dir="ltr">Is self-sufficiency the best solution?</h3>
After the supply price increase, the price of a ton of local wheat <a href="https://www.tridge.com/news/grain-room-the-price-of-a-ton-of-local-wheat-is-10">stands</a> at EGP13,300, compared to EGP12,200&ndash;12,500 for imported wheat. This discrepancy is a result of the state&rsquo;s years-long policy of bumping up the supply price for domestic wheat. In fact, in 2014, a ton of <a href="https://www.fao.org/3/i4898e/i4898e.pdf">locally grown wheat</a> was going for USD84 more than imported wheat. The efforts to increase the local production of wheat remain important to ensure a degree of <a href="https://aps.aucegypt.edu/en/articles/1253/food-security-and-green-hydrogen-who-benefits">food security</a> in Egypt, especially considering the declining per capita consumption of <a href="https://aps.aucegypt.edu/en/articles/1368/lack-of-transparency-and-protein">vegetable protein</a>, of which <a href="https://aps.aucegypt.edu/en/articles/1017/fdi-and-wheat-gap-challenges">wheat</a> is a chief source. Poor Egyptians also <a href="https://www.frontiersin.org/articles/10.3389/fsufs.2020.617009/full">depend</a> on wheat because it is widely available and relatively affordable thanks to government subsidies.&nbsp;

Despite the importance of self-sufficiency, attempts to <a href="https://ejar.journals.ekb.eg/article_231078_58561a54b79dd30fc5d6766836ae9665.pdf">sustain</a> reasonable sufficiency rates <a href="https://www.fao.org/3/i4898e/i4898e.pdf">must be tempered</a> with attention to other crops that add value to the economy, such as vegetables, which can be exported. They could also be made more widely available at affordable prices, which may encourage citizens to eat a more diverse diet, thereby reducing dependence on wheat products that cause illnesses like diabetes.
