<p style="text-align: justify;">ثانيًا: إذا اعتبرنا أن هذا المقترح التشريعي بمثابة تدشين مبدأ الشراكة بين القطاعين العام والخاص في مجال الخدمات الصحية (<a href="https://www.pwc.com/gx/en/healthcare/assets/ppps-in-healthcare.pdf">PPPs</a>)، فإنه لم يتطرق بشكل واضح إلى آليات تنفيذ هذه الشراكة المعمول بها في الدول الأخرى، وأهمها أن تكون قيمة الإنفاق الشخصي على العلاج في المستشفيات العامة أقل أو مساوية لما كان عليه الحال قبل طرح إدارتها للقطاع الخاص. وهو ما قد يزيد العبء المالي على المواطنين، خاصة وأن <a href="https://sis.gov.eg/Story/257946/%D9%88%D8%B2%D9%8A%D8%B1%D8%A7-%D8%A7%D9%84%D8%B5%D8%AD%D8%A9-%D9%88%D8%A7%D9%84%D9%85%D8%A7%D9%84%D9%8A%D8%A9-%D9%8A%D8%B7%D9%84%D9%82%D8%A7%D9%86-%D8%AA%D9%82%D8%B1%D9%8A%D8%B1-%D8%A7%D9%84%D8%AD%D8%B3%D8%A7%D8%A8%D8%A7%D8%AA-%D8%A7%D9%84%D9%82%D9%88%D9%85%D9%8A%D8%A9-%D9%84%D9%84%D8%B5%D8%AD%D8%A9-%D9%81%D9%8A-%D9%85%D8%B5%D8%B1?lang=ar">الإنفاق الشخصي</a> يمثل بالفعل حوالي 60% من إجمالي الإنفاق على الصحة.</p>
<p dir="rtl" style="text-align: justify;">ثالثًا: أغفل المقترح مصير الأطباء وأطقم التمريض ومختلف العاملين في تلك المنشآت العامة في حال قرر المستثمر الخاص تقليل العمالة من أجل تضخيم العائد المالي. يرى محمود فؤاد المدير التنفيذي لجمعية الحق في الدواء أن هذا الأمر من شأنه أن يؤدي إلى ارتفاع معدلات البطالة بين العاملين في القطاع الصحي من المصريين، وقد يزيد من <a href="https://www.ems.org.eg/ar/news/Committees/2430?fbclid=IwAR3rCqTgo7gwX8t9-ZwWDTvzxDZsn8tCA1wwHdfpIMDUWUXnR60rG48nuqM">أزمة هجرة الأطباء</a>.</p>

<p dir="rtl" style="text-align: justify;">زادت في المقابل نسبة إجمالي الاستثمارات المحلية في قطاع خدمي مثل النقل والتخزين من 10.6% إلى 22.1% خلال نفس الفترة. تمثل الاستثمارات في قطاع الصناعة قيمة مضافة أعلى من مثيلتها في قطاع النقل والتخزين، من حيث دعم الاقتصاد الوطني وتوظيف عمالة دائمة وماهرة، لكن نسبة الاستثمارات الأجنبية لقطاع الصناعة <a href="https://www.cbe.org.eg/ar/economic-research/economic-reports/monthly-statistical-bulletin">بلغت</a> 33.3% من الإجمالي في عام 2023/2022 مقارنة بـ 59.2% موجهة إلى القطاع الخدمي.&nbsp;</p>

<p dir="ltr" style="text-align: justify;">First and foremost, the government bill does not incorporate explicit conditions for pricing health services when it transfers the management of a public facility to the private sector. This condition is crucial to prevent investors from inflating the prices to maximize their returns. Medical institutions that fall under the jurisprudence of the Ministry of Health <a href="https://www.capmas.gov.eg/Pages/Publications.aspx?page_id=5104&amp;Year=23361">constitute</a> about 80% of all government hospitals that should be offering free or tokenary priced services. Public hospitals serve more patients, even though their total number falls behind private ones.&nbsp;</p>
<p dir="ltr" style="text-align: justify;">Secondly, the bill does not provide for the kind of implementation requirements that are in place in other countries that have undertaken <a href="https://www.pwc.com/gx/en/healthcare/assets/ppps-in-healthcare.pdf">public-private partnerships</a> in health services. . Most importantly, the value of out-of-pocket spending on treatment in privately managed public hospitals should be equal to or less than the value before privatization. Unregulated pricing could impose additional financial burdens on citizens, especially considering that <a href="https://sis.gov.eg/Story/257946/%D9%88%D8%B2%D9%8A%D8%B1%D8%A7-%D8%A7%D9%84%D8%B5%D8%AD%D8%A9-%D9%88%D8%A7%D9%84%D9%85%D8%A7%D9%84%D9%8A%D8%A9-%D9%8A%D8%B7%D9%84%D9%82%D8%A7%D9%86-%D8%AA%D9%82%D8%B1%D9%8A%D8%B1-%D8%A7%D9%84%D8%AD%D8%B3%D8%A7%D8%A8%D8%A7%D8%AA-%D8%A7%D9%84%D9%82%D9%88%D9%85%D9%8A%D8%A9-%D9%84%D9%84%D8%B5%D8%AD%D8%A9-%D9%81%D9%8A-%D9%85%D8%B5%D8%B1?lang=ar">out-of-pocket spending</a> already constitutes a significant portion of total health spending, amounting to 60%.</p>
<p dir="ltr" style="text-align: justify;">Thirdly, the bill overlooks the crucial issue of the fate of healthcare workers, including doctors, nurses, and other staff in these public facilities. If the private investor decides to cut the workforce for higher profits, it could lead to significant layoffs. Mahmoud Fouad, executive director of the Right to Medicine Association, echoes this concern. He believes the bill would lead to higher unemployment among Egyptian health workers and may exacerbate the already critical level of <a href="https://www.ems.org.eg/ar/news/Committees/2430?fbclid=IwAR3rCqTgo7gwX8t9-ZwWDTvzxDZsn8tCA1wwHdfpIMDUWUXnR60rG48nuqM">emigration by doctors</a>, causing a medical brain drain.</p>
<p dir="ltr" style="text-align: justify;">Fourthly, the bill constitutes another instance of the government's attempt to reduce public spending on health. Since 2016, government spending on the health sector as a percentage of GDP has consistently fallen below the constitutional minimum threshold of 3%. This meager spending is a clear violation of the Egyptian Constitution. It underscores the need for policymakers to prioritize health spending to ensure citizens can access quality medical services.</p>

The government should also increase the number of hospital beds and establish new public hospitals that provide every citizen with medical care that meets international standards.</li>

While government support of the industrial sector is vital to boosting employment, GDP, and foreign currency from exports, the value and increase of finished and semi-finished goods exports remain relatively low. This is mainly due to the government&rsquo;s neglect of sustainable local industries, which provide skilled professional jobs and export high-value-added products.</p>

<h3 dir="ltr"><strong>Government gives service sector precedence over industry</strong></h3>
<p dir="ltr" style="text-align: justify;">Local investments directed to the industrial sector <a href="https://www.cbe.org.eg/ar/economic-research/economic-reports/monthly-statistical-bulletin">fell</a> from 15.1% in 2013/14 to 4.7% in 2022/23, although it is vital for supporting domestic production and manufacturing exports.&nbsp;</p>

<p dir="ltr" style="text-align: justify;">In contrast, local investments in services such as transport and storage increased from 10.6% to 22.1% of the total during the same period. While investments in the industrial sector have a higher added value to the economy and build a more robust and skilled labor force, only <a href="https://www.cbe.org.eg/ar/economic-research/economic-reports/monthly-statistical-bulletin">33.3%</a> of foreign investments went to the industrial sector in 2022/23, compared to 59.2% to the service sector.</p>

<h3 dir="ltr"><strong>Industries leading the pack</strong></h3>
<p dir="ltr" style="text-align: justify;">The trade balance <a href="https://www.cbe.org.eg/ar/economic-research/economic-reports/monthly-statistical-bulletin">shows</a> that the textile sector is the only one that has achieved a surplus. The industry went from recording a deficit of USD309 million in the first quarter of 2014/15 to a surplus of USD108 million in the same quarter of 2023/24.</p>
<p style="text-align: justify;">Egyptian industries <a href="http://www.library.idsc.gov.eg/cgi-bin/koha/opac-detail.pl?biblionumber=48968">produce</a> goods in high global demand, such as chemicals, wood, paper, processed food, and agricultural products. The textile industry <a href="http://www.imc-egypt.org/index.php/ar/2020-12-14-09-32-25/2020-12-30-06-04-7">leads</a> the pack, however, and it is not only because it has a competitive advantage and more export opportunities. It is also a labor-intensive sector, employing 25% of the industrial workforce and a technology-intensive industry. It comprises 12,000 manufacturing firms and workshops with total investments of some EGP300 billion.</p>
<h3 dir="ltr"><strong>Lessons learned from textiles to plug the budget deficit</strong></h3>
<p dir="ltr" style="text-align: justify;">First, given the paltry increase in the export of finished and semi-finished manufactured goods over the last decade, the government should focus on boosting the industrial sector. It should do so by bridging the investment gap between the industrial and services sectors, stopping its prioritization of the latter.</p>
<p dir="ltr" style="text-align: justify;">Second, the government should inject new investments into industries that contribute to sustainable development, such as textiles and others that localize knowledge, boost technology and provide highly skilled professional jobs. These industries can revive local manufacturing and reinvigorate foreign trade, which will help plug the budget deficit in the medium and long terms.</p>
