The government healthcare spending has <a href="https://aps.aucegypt.edu/en/articles/1391/egypts-public-hospital-for-sale-and-its-robust-industry-out-of-focus">consistently fallen below</a> the 3% of GDP constitutional mandate, leaving public medical institutions to decay.&nbsp;
Hospitals are also understaffed, leading physicians to take on non-medical administrative duties. Overcrowded hospitals, excessive patient loads, and administrative mismanagement have compounded the stress on doctors. In 2022, the Egyptian Medical Syndicate <a href="https://www.scidev.net/mena/news/egyptian-physicians-are-emigrating-and-awaiting-right-opportunities/">recorded</a> 149 doctor deaths from work exhaustion.&nbsp;

Doctors are increasingly fed up with what they refer to as a hostile working environment and declining respect for their profession among the public. A <a href="https://www.researchgate.net/publication/376585668_Defensive_medicine_practices_as_a_result_of_malpractice_claims_and_workplace_physical_violence_a_cross-sectional_study_from_Egypt">2023 study</a> revealed that over 70% of Egyptian doctors experienced some form of <a href="https://aps.aucegypt.edu/en/articles/1423/addressing-violence-in-hospital-emergency-rooms">workplace violence</a>.&nbsp;

Finally, limited career development opportunities drive many Egyptian doctors to seek work abroad. Public hospitals lack structured training programs and effective performance oversight. Pursuing specialization requires either enrolling in limited Ministry of Health programs after two years of compulsory service or self-funding a master's degree. In a <a href="https://fount.aucegypt.edu/cgi/viewcontent.cgi?article=3465&amp;context=etds">study of doctors working abroad</a>, around 53% of physicians cited a lack of training opportunities and 54% cited unclear career prospects among their main reasons for migration.

<h3 dir="ltr" style="text-align: justify;">Addressing the structural causes of medical emigration</h3>
The government must address the underlying structural factors driving doctors to emigrate. It should prioritize raising salaries in the public health sector to meet the actual minimum cost of living. This must be coupled with academic scholarships and clear guidelines for career advancement. It is also essential for the government to improve the hospital work environment by ensuring the availability of basic equipment and increasing the healthcare budget to at least meet the constitutional requirement. In addition, the government should develop clear protocols to protect physicians from workplace violence and exhaustion. Notably, a 2021 <a href="https://applications.emro.who.int/EMHJ/V27/11/1020-3397-2021-2711-1102-1108-eng.pdf?ua=1">study</a> found that 97.4% of doctors considering emigration would reconsider if the sector significantly improved.
In parallel, the government should prioritize optimizing the experience and skills of those who choose to work abroad through programs facilitating channels of knowledge exchange. The government can do this by establishing a national body dedicated to engaging the Egyptian medical diaspora and supporting their contributions to the development of the local healthcare system.&nbsp;
