<p style="text-align: justify;">تتنوع التحديات التي تواجه قطاع الصحة في تحقيق الرعاية الصحية الشاملة؛ وتشمل عدم كفاية التمويل وارتفاع أسعار المنتجات الصحية والتدخلات السياسية غير الفعالة في مجال الأدوية الجنيسة والبدائل الحيوية (World Health Organization 2019). وتقدر منظمة الصحة العالمية أنه من أجل تحقيق أهداف التنمية المستدامة (SDGs) بشأن الرعاية الصحية الشاملة (UHC) في البلدان المتوسطة الدخل من الشريحة الدنيا، من الضروري توفير 370 مليار دولار أمريكي إضافية سنويًا للرعاية الصحية الأولية (Organisation for Economic Co-operation and Development ، 2020). وبالإضافة إلى ذلك أدى عدم الحصول على الأدوية بأسعار معقولة إلى تفاقم تدهور جودة المنتجات الطبية فضلاً عن زيادة الإنفاق من الجيب على الرعاية الصحية (WHO &amp; World Bank, 2019). ويُدفع ما يقرب من 70% من إجمالي الإنفاق الصحي في مصر من الجيب، وتمثل مشتريات الأدوية من الصيدليات المجتمعية ما يقرب من نصف هذه النفقات من الجيب الخاص (Mohamed &amp; Kreling, 2016).&nbsp;</p>
<p style="text-align: justify;">وتعتبر مصر من بين العديد من الدول التي لديها القدرة على تصنيع الأدوية لكل من المواد الفعالة والمنتجات التامة الصنع باستخدام المكونات المستوردة (Ministry of Health، 2019). ويغطي الإنتاج المحلي من الأدوية من المصانع الخاصة والعامة في مصر أكثر من 80% من استهلاكها المحلي بينما تغطي الأدوية المستوردة 20% (Ministry of Health، 2019). ووفقا لتقرير التنافسية العالمي الصادر عن المنتدى الاقتصادي العالمي، شغلت مصر المرتبة الثالثة والتسعين من بين 140 بلدا، مما يجعلها في الرُبيع الأدنى للبلدان (WEF, 2019). ومثل إنفاق مصر على البحث والتطوير من الناتج المحلي الإجمالي 0.6% فقط، أي أقل بنسبة 20.2 نقطة مئوية مقارنة بنتائج تقرير عام 2017؛ وعلى مقياس يتراوح من 0 إلى 100 حققت مصر مرتبة 0.05 في مكانة الجامعات والمؤسسات البحثية البارزة في مجال البحث والتطوير (WEF, 2019). وبشكل عام، تحتاج منطقة الشرق الأوسط وشمال إفريقيا حاجة ماسة إلى تعزيز قدرات البحوث الصحية، نظرًا لأنها لا تساهم سوى بنسبة 1.5% في الإصدارات والأبحاث العالمية السنوية (El Achi et al., 2019).&nbsp;</p>
<p style="text-align: justify;">وفي سبتمبر 2019 أعلنت الحكومة المصرية عن صياغة خطة وطنية لدعم قطاع الأدوية مشيرة إلى تنامي إمكانات مشاركة الأدوية المصرية في الأسواق الدولية (Egypt Today, 2019). وتتضمن هذه المبادرة التركيز على تعزيز صادرات الأدوية وبالتحديد التركيز على أسواق الأدوية الأفريقية بناءً على الإطار الإقليمي بموجب معاهدة إنشاء الوكالة الأفريقية للأدوية (الشرق الأوسط، 2019). ومع ذلك، لم تُعلن بعد نتائج هذه الخطة الوطنية للجمهور. وبالنظر إلى كل ما سبق تتزايد الحاجة لضمان توفر المنتجات الصحية بشكل كاف ومستدام من خلال سياسات تضمن خدمات صحية ميسورة التكلفة، بما في ذلك الأدوية.</p>

<p style="text-align: justify;">The challenges facing the health sector in achieving universal healthcare are varied; they include inadequate financing, high prices of health products and ineffective policy interventions for generic and biosimilar medicines (World Health Organization [WHO], 2019). The WHO estimates that, in order to achieve the Sustainable Development Goals (SDGs) targets on universal healthcare (UHC) in low- and middle-income countries (LMICs), an additional USD370 billion per year is needed for primary healthcare (Organisation for Economic Co-operation and Development [OECD], 2020).&nbsp;In addition, lack of access to affordable medicines has exacerbated the dwindling quality of medicinal products, as well as increased out-of-pocket (OOP) expenditure (WHO &amp; World Bank, 2019). Approximately 70% of the total health expenditure in Egypt are paid OOP, and drug purchases at community pharmacies represent nearly half of these out-of-pocket expenses (Mohamed &amp; Kreling, 2016).&nbsp;</p>

<p>Egypt is considered among one of many countries with the pharmaceutical manufacturing capacity of both active ingredients and finished products from imported ingredients (Ministry of Health [MoH], 2019). Egypt&rsquo;s local production of medicines, private and public industry covers more than 80% of its domestic consumption, whereas imported medicines cover 20% (MoH, 2019). Research and Development (R&amp;D) in this industry is relatively weak; according to the World Economic Forum&rsquo;s (WEF) Global Competitiveness Report,&nbsp;In 2019 Egypt ranked 93rd out of 140 countries, marking it as amongst the lowest quartile of countries (WEF, 2019). Egypt&rsquo;s R&amp;D expenditure out of GDP was only 0.6%, 20.2 percentage points lower than the 2017 report findings, and on a scale from 0 to 100, Egypt achieved a rank of 0.05 in university and research institutions&rsquo; prominence in R&amp;D (WEF, 2019). Overall, the MENA region is in urgent need of health research capacity strengthening, given that it only contributes 1.5% of annual global publications and research (El Achi et al., 2019).&nbsp;</p>

<p>In September 2019, the Egyptian government announced the drafting of a national plan to support the pharmaceutical sector, pointing to the growing potential of Egyptian medicines&rsquo; participation in international markets (Egypt Today, 2019). This initiative includes a focus on promoting medicine exports, specifically with a focus on African pharmaceutical markets, based on the interregional framework under the Treaty for the Establishment of the African Medicine Agency (AMA) (Asharq Al-Awsat, 2019).&nbsp;However, the details of this national plan have not yet been announced to the public. Given all of the above, there is an increasing need to ensure the adequate and sustainable availability of health products through policies that guarantee affordable health services, including medicines.</p>

<div style="text-align: justify;">
<p>This background paper is concerned with assessing the Egyptian pharmaceutical sector, its global position and its potential to make medicines more accessible by boosting the local generic industry. The qualitative research is based on a desktop review of the literature, publicly available data on the pharmaceutical industry and a review of the international framework governing intellectual property, which includes the flexibilities provided to developing countries. The literature is summarized in the first sections of the paper, which concludes with a brief analysis of the main policy challenges the Egyptian pharmaceutical sector faces in providing accessible medicines.&nbsp;</p>

<p>The paper begins with a section on key terminology of the pharmaceutical domain. This is followed by a literature review of the production of medicine and Intellectual Property Rights (IPR) in the Egyptian context. The subsequent section is devoted to the policy context and governing framework, providing a comprehensive view of macro-level policies governing global and Egyptian pharmaceutical sectors. This section is particularly dedicated to assessing the impact of price control regimes on access to medicine, in order to aid policymakers in formulating more inclusive methods to foster sustainable access to health, as well as boost the pharmaceutical industry. The concluding section of this paper assesses the challenges impeding equitable access to medicine and expansion of the generic production of medicines in the pharmaceutical sector in order to find areas for effective policy intervention. The findings highlight the potential role of pharmaceutical manufacturing capacities in making medicine more affordable and propose possible opportunities to improve access to medicine.</p>
